| If your child has been attending another Clark County School District school, you will need to provide the following:
Fill out the Registration form: English
or Spanish
For proof of address, you must bring one of the following:
- recent gas, power, or water bill
- rental agreement
- residential lease or sales contract
Note: If you have a sales contract for the purchase of a new home as proof of address, the school will enroll your child. To obtain the second proof of address you can call the utility company at the numbers listed below and have them fax your turn on order to us at 799-7241.

Driver's licenses, telephone bills, or cable bills are not acceptable. If you are living with a friend or relative, you must:
- come in to the front office to get a Residential Affidavit form. It is to be signed by you and the owner of the residence in front of a notary to indicate that it is your place of residency; and
- bring your personal identification (Nevada driver's license, government picture I.D., etc.) which bears the address of the friend or relative with whom you reside.

If you are new to the Clark County School District, you must also provide, in addition to those items listed in the first column:
For proof of your child's identity, please bring one of the following (photocopies will not be accepted):
- original birth certificate
- passport
- certified birth card (issued by the Clark County Health District or another health district)
For proof of immunizations, you must have records indicating that your child has had, or has at least started, this series of immunizations:
- 5 DPT shots
- 4 Polio shots
- 2 MMR shots
- 2 Hepatitis A shots
- 3 Hepatitis B shots
- Chicken pox (Varicella) shot
